Investigators have released a photo of the dark-colored minivan police said struck and seriously injured a 25-year-old man in a hit-and-run crash last month in Central Islip.

Now, Suffolk County police are asking anyone who may recognize the van -- or who may have information about the driver or the crash -- to call Crime Stoppers at 800-220-TIPS or Vehicular Crime Unit detectives at 631-852-6555. All calls will remain confidential, police said.

The crash occurred near the intersection of Carleton and Brightside avenues at about 9:25 p.m. on Aug. 17.

The victim, identified by police as Edgar Abrego of Central Islip, suffered head and body injuries and was taken to Stony Brook University Hospital for treatment. The status of his recovery was not immediately clear on Wednesday.

Police said the van suffered damage to its driver's side mirror -- and, possibly, to the front windshield.

Police also are seeking a Good Samaritan in a white sedan who stopped at the scene, backing up to protect Abrego from being struck by other vehicles, but who left the scene before being interviewed by responding detectives.

It was not clear whether detectives have been able to trace a license plate on the van -- or, the Good Samaritan's vehicle -- using surveillance footage.
