Monday will mark six months since superstorm Sandy hit Long Island.

As part of our continuing Sandy coverage, we are compiling a list and map of businesses that have reopened, accompanied by brief stories.

The idea is two-fold: To remind customers that local businesses are back, and to create a more robust history of Sandy's impact.

If your business was affected and has reopened, please fill out the following information and you could be included.
