When I heard about the death of 2-year-old Greta Greene after debris fell on her from the facade of an Upper West Side building, my heart was filled with sadness ["Girl, 2, struck by falling bricks dies," News, May 19].

As a grandfather, I can only imagine the anguish and sorrow of her grandmother, who was sitting on a bench enjoying a wonderful Sunday morning cradling the love of her life. It seems that this tragic accident might never have happened had the city Department of Buildings followed up on a fine that was issued because a required building facade report wasn't filed in 2011.

Issuing a violation to the building's owner after the accident is too little, too late.
