iPatientCare, Inc. takes pride announcing its practices up and running successfully with its interface implemented for MiPCT (Michigan Primary Care Transformation Project) for The Michigan Data Collaborative (MDC) to transform healthcare and provide superior primary care to patients especially those with multiple chronic diseases

Detroit, MI (PRWEB) June 21, 2016

iPatientCare, a pioneer in cloud-based ambulatory EHR and Revenue Cycle Management services, is glad to announce its successful implementation of interface for Michigan Primary Care Transformation Project (MiPCT) for the Michigan Data Collaborative (MDC) in order to enable its users becoming participating practices to enhance analysis of multi-payer claims data, enable actionable care plans, and monitor and improve quality measures.

The Michigan Primary Care Transformation Project (MiPCT) aimed at improving health in the state, making care more affordable, and strengthening the patient-care team relationship. This transformation happens through a collaborative network of POs/PHOs and shared learning opportunities where the Michigan Data Collaborative (MDC) plays an important role. MDC is providing online, dynamic dashboards and reports for Physician Organizations (POs) designed for participating providers to enhance analysis of multi-payer claims data, enable actionable care plans, and monitor and improve quality measures.

With help of Northern Physician Organization (NPO), iPatientCare has achieved this interface. This implementation will help iPatientCare users to submit the data which is used in the registry at the MDC to rank physician organizations with MiPCT participating practices and pay an incentive. The data submitted is clinical data combined with claims data to help create a dashboard for the MiPCT participating organizations so that they can better track clinical quality and further expand the capabilities of the PCMH (Patient Centered Medical Homes) model in Michigan.

“We have been very satisfied with the work iPatientCare has done on both this project and others involving NPO’s Health Information Exchange. Their prompt and thorough service has allowed us to connect their electronic health record system to our HIE, send their practices hospital notifications, and now submit MiPCT quality reporting. We anticipate working with on many projects in the future,” said Ed Worthington, IT Director – Northern Physicians Organization.

“iPatientCare strives to make healthcare IT work efficiently at reduced cost. With constant innovations and achievements, we have helped physician practices improve efficiency with the best service to their patients. This is an important step toward care coordination management where POs/PHOs involved at the community level ultimately contribute to expands access to primary care for patients especially those with multiple chronic diseases,” said Milan Kalola, Project Manager - Clinical Implementation, iPatientCare.
