"Modern Family" actress Sarah Hyland underwent a kidney transplant last month to repair a congenital condition called kidney dysplasia.

Hyland, 21 -- who plays the elder Dunphy daughter, Haley, on the hit ABC comedy -- received a kidney from her father, Edward James Hyland, in an operation on April 13.

"You know that family is always going to be there for you -- no matter what," she told Seventeen magazine in its June/July issue, due out May 22. "My dad gave me a freakin' kidney!"

Hyland, who was diagnosed with the ailment when she was 9, gave the interview while recovering from surgery.

She said her boyfriend, "High School Musical 3" actor Matt Prokop, "helps me with my medication and takes me to doctor appointments and all the stuff that I need to do to take care of myself. Any other 21-year-old guy would not be there, I guarantee it. I am one of the luckiest people in the world to have him be there for me."

Prokop has also appeared as Haley's beau on a "Modern Family" episode.

Hyland told "Good Morning America" Monday that "I would be in a lot of pain a lot of the time" while working on her show. "You know, if you're sick, you still go to work. And in between takes, you sit down or you lay your head down or something."

The experience, she told Seventeen, has left her grateful. "I have a second chance at life -- not a lot of people get that."
