A Roslyn man was arrested Tuesday after presenting a fraudulent prescription for oxycodone at a Hicksville pharmacy twice in October, police said.

Jason Kang, 30, of 78 Woods Dr., surrendered at 10 a.m. to Nassau police, according to police.

He was arraigned at First District Court in Hempstead on two counts of criminal possession of a forged instrument, police said.

Kang was conditionally released to probation and is scheduled to return to court Feb. 18, according to online court records. His attorney's information was not immediately available.

Kang entered Smith Pharmacy, 53 North Broadway, on Oct. 7 and 8 to fill a prescription for oxycodone, according to police.

A pharmacy employee verified Kang's identification with his driver's license, but on both occasions the prescription had been altered, according to police.

Kang fled the scene each time, police said.
