A truck driver was indicted Tuesday on a charge of leaving the scene of an October hit-and-run accident in Holbrook that seriously injured a bicyclist, the Suffolk district attorney's office said.

John Farina, 50, of Ronkonkoma, was charged with leaving the scene of an accident involving an injury, a felony, and speeding, an infraction.

The grand jury indictment was unsealed by State Supreme Court Justice Fernando Camacho in Central Islip.

Farina was arrested Oct. 24, after a box truck hit Bradley J. Samuel at about 9 p.m. on Oct. 6 as he was bicycling eastbound on Veterans Memorial Highway, west of Coates Avenue, police said.

At the time of Farina's arrest, police said his truck had been impounded and that parts recovered at the accident scene -- including an insert from the passenger-side mirror -- were matched to the truck by the Suffolk County crime lab.

He has been free since posting $5,000 cash bail on Oct. 24, according to court records.
