CENTER MORICHES - A Center Moriches Middle School teacher who was arrested after allegedly driving under the influence of drugs and alcohol in a hit-and-run crash was arraigned Tuesday.

Tracey Forman, 47, was charged with leaving the scene of an accident and driving while ability impaired by a combination of drugs and alcohol. 

Police say Forman was driving on Brookfield Avenue on her way to work when she hit a parked minivan and then took off on Monday.

Authorities say they located Forman and her vehicle a short time later in a parking lot behind Center Moriches High School. That building is connected to the middle school where Forman is a teacher. 

Court documents reveal Forman thought she hit a pole, but was not sure. The documents also revealed Forman admitted she should not have been driving. She also admitted “taking two shots” the night before the accident and taking anti-anxiety medication in the morning.

In addition, documents revealed that Forman had a prior arrest back in December for allegedly stealing from Kohl’s. 

Bail was set for Forman at $2,500 cash, $5,000 bond. She is due back in court next month.

Forman's attorney and the Center Moriches School District had no comment on the arrest.
