Actress Demi Moore is seeking professional help for unspecified health issues, her representative said Tuesday, following a report that she was hospitalized Monday night.

"Because of the stresses in her life right now, Demi has chosen to seek professional assistance to treat her exhaustion and improve her overall health," her spokeswoman said in a statement to Newsday. "She looks forward to getting well and is grateful for the support of her family and friends."

After TMZ.com reported that paramedics transported Moore, 49, to a Los Angeles hospital Monday night, Los Angeles City Fire Department spokesman Matt Spence told People magazine, "I can confirm that we received an emergency call at 10:49 p.m. for an unknown illness related to [Moore's] address. We transported at least one patient to a local hospital. The patient was transported at 11:21 p.m."

A spokeswoman for Moore's estranged husband, actor Ashton Kutcher, from whom Moore separated in November, did not respond to a request for comment.

Moore is set to cameo as feminist Gloria Steinem in "Lovelace," the biography of iconic 1970s adult-film star Linda Lovelace. The movie, which stars Amanda Seyfried, is shooting in Los Angeles. Moore also has a development deal with Lifetime Television for three new series including the talk show "The Conversation," hosted by British TV personality Amanda de Cadenet.
