A Farmingdale woman has been arrested after she “forged electronic prescriptions” for Percocet while working as an assistant for a Hicksville doctor, police said Thursday.

Katarina Tountas, 34, of Conklin Street, is scheduled to appear in court Friday after being ordered held on $1,000 bond or cash bail.

Tountas was arrested Tuesday following an investigation by narcotics/vice squad detectives. She was arraigned Wednesday on six counts of second-degree forgery, records showed.

Court records show that Tountas, who police said was employed as medical assistant from August 2015 until February 2016, the time the alleged forgeries took place, previously pleaded guilty to drug charges — and had been released to probation.

The records showed that after that guilty plea in 2013 Tountas was ordered to complete 50 hours of community service, had her medical license suspended for six months, was given 3 years of probation and was fined $1,000. Records show she was paying that fine on installments before the new arrest took place this week.

Investigators said the between August and February, Tountas used a computer to forge prescriptions on six different occasions — transmitting them to a local pharmacy.

Records indicate that after her arraignment Wednesday, Tountas failed to make bail.
