A Jericho man was arrested Tuesday after police said he tried to use a forged prescription to obtain Vicodin from a pharmacy in Woodbury in May.

Melvin Pagan, 30, of Dawson Lane, surrendered to Nassau County police at the Second Precinct and was charged with 12 counts of second-degree possession of a forged instrument, police said. He is scheduled for arraignment Wednesday in First District Court in Hempstead.

Police said Pagan entered the Walgreens pharmacy on South Oyster Bay Road on May 22 and attempted to pass a forged prescription. But, police said, the pharmacist identified the fake and notified them -- then checked the pharmacy database and discovered other fraudulent prescriptions for Vicodin that had been filled for Pagan.
