Police have issued a Silver Alert for a Valley Stream woman who was last seen Monday evening on a street in Brooklyn.

Nassau County police said Elizabeth Perez, 21, is in good physical health, but had no money.

Perez, who is white, 5-foot-2 and 140 pounds with brown hair and brown eyes, was last seen at about 6 p.m. near the intersection of Atlantic Avenue and Rockaway Avenue, police said.

She was wearing a gray, short-sleeved shirt and blue jeans.

A Silver Alert is issued by police when a missing person suffers from Alzheimer's disease, dementia or some other type of cognitive disorder.

Anyone with information can contact police at 516-573-8800. All calls will be kept confidential, police said.
