Police have asked for the public’s help in finding a homeless man who has not been seen since he was discharged from a hospital in December.

Thomas Eastwood, 58, was discharged Dec. 14 from St. Catherine of Siena Medical Center on Route 25A in Smithtown, police said, and his brother reported him missing on Dec. 30.

Eastwood was described as 5 feet, 11 inches tall and weighing about 250 pounds.

Detectives ask anyone with information to call 911 or Second Precinct detectives at 631-854-8252.
