Four workers should be disciplined over derailment, MTA report says

Transit officials say a New York City subway derailment this year that injured 30 riders and two employees was caused by track defects.

A Metropolitan Transit Authority report released Friday finds that inspectors didn't document and investigate a broken plate, fasteners and a deteriorated tie on the Queens subway line.

The report says a combination of defects caused the May 2 derailment of an eight-car train that caused $2 million in damage. About 1,000 commuters were on board.

The report recommends discipline against three maintenance supervisors and a track inspector.

Transport Workers Union Local 100 President John Samuelsen says in a statement the MTA report's recommendation for discipline is scapegoating. He says the rail was defective.
